I used to see one in Winchester, VA, but it always looked closed.

Grill Dome ceramic / Ducane Affinity 4200 gasser/ Concrete pit
We had on in Rockville, MD when I was a kid in the 60's and 70's. Think I only went there once or twice after I got my drivers license and then it close.
The pit was a brick thing with metal doors and the fire bow was outside 20 feet away. Don't know it that supplied all of the heat as well or just the smoke.
